Forms

Your application for registration (ie policies and procedures against criteria) must also include each of the following forms.

Queensland statutory declaration (new window) 93k Adobe PDF document
Delegated responsibility (optional) (new window) 53k Adobe PDF document
Request to register new course (one for each course) (new window) 218k Microsoft® Word document
Minor changes to provider details form (new window) 232k Microsoft® Word document
Changes to PEO or higher managerial agent, ownership, Trading Name or contact details.
Major changes to provider details form (new window) 271k Microsoft® Word document
Changes to teaching premises and Increases in student capacity.

New Provider Registration

You can now submit your registration online (see below). Alternatively, for an application pack, please contact our office on 07 3237 1883.

Cancel registration

If you wish to cancel your registration as a CRICOS provider, please submit a cancel provider form (new window) 144k Microsoft® Word document to the State Authority. These can be faxed. To cancel registration of individual courses, please go to COURSES.

Fees

Applications for new registration should be accompanied by the registration fee of $245.00. Registration is for one calendar year. The fee for renewing registration is also $245.00.

Credit card payments can be made on the payment form (new window) 46k Adobe PDF document. Cheques should be made out to the Department of Education, Training and the Arts.

Online Form

You can now use the online form for New Provider Applications and New Course Applications (for registered providers).
