School transport assistance program - forms for departmental use

If you are an employee of the Department of Education and Training with access to OnePortal, you are able to access the following on OnePortal:

Letters to parent/carers (DET employees only):

M1 Letter advising parent/carer/guardian of authorisation of requested transport assistance
M2 Letter advising parent/carer/guardian of the approval of a different type of transport assistance than that requested
M3 Letter advising parent/carer/guardian that transport assistance application is not approved

Appeals process forms and letters (DET employees only):

A3 Appeal process - rationale for decision re school transport assistance
A5 Appeal process - letter to parent/carer/guardian from Assistant Regional Director

Claim form for payment to Teacher Aides as student escorts (DET employees only):
