The EAP Consent Form (EAP 1) updated 2015 formally documents agreement between the parent and the school for verification, EAP profile and reviews. This form is kept in the student's school file and must be uploaded as an attachment to the verification request on AIMS in OneSchool.
Schools can submit completed verification requests at any time during the year. Verification reviews should be completed as close as possible to the review dates recorded on AIMS in OS.
