Education Adjustment Program

The EAP has three components:

Verification requests - State schools

All verification requests are submitted through AIMS in OneSchool.

Verification Requests - Non-state schools

Verification requests in the EAP categories of hearing impairment, physical impairment and vision impairment are submitted via email to the category-specific inbox. For forms related to the categories, please refer to the EAP verification forms page.

EAP consent Form (EAP 1) for parents

The EAP Consent Form (EAP 1) updated 2015 Adobe PDF document formally documents agreement between the parent and the school for verification, EAP profile and reviews. This form is kept in the student's school file and must be uploaded as an attachment to the verification request on AIMS in OneSchool.

Verification requests

Schools can submit completed verification requests at any time during the year. Verification reviews should be completed as close as possible to the review dates recorded on AIMS in OS.
