Students Previously Identified in Non-State Schools

Some non-state school sectors have processes for identifying students with a disability in the equivalent EAP disability categories of Autism Spectrum Disorder, Intellectual Impairment and Speech-Language Impairment. For students who have been formally identified in a non-state school in Queensland within the last two years (through an EAP equivalent process) the school team can use the existing information for a verification request and:

obtain parent permission to proceed using the EAP Consent Form (EAP 1) (to be kept in the school file)
register the student on AIMS
complete PART A of the EAP Verification Form (Student Details)
collate the existing formal documentation from the previous school (e.g. signed forms and appropriate reports)
document any additional information required to meet relevant DET criteria
ensure the appropriate Quality Assurance section of the EAP Verification Form (for intellectual impairment and speech-language impairment) is completed (according to local quality assurance processes)
complete PART C (Principal Request for Verification)
submit the verification documents to the EAP Verification Team.

The provided information must meet current DET criteria for the EAP disability category. The statewide verifier will consider the information and enter a verification decision on AIMS, including a review date if required.
