Formal agreement between the parent and the school team is required before proceeding through the EAP process. The EAP Consent Form (EAP 1 updated 2009) (new window) Microsoft® Word document must be used to document this agreement. The original of this form must be kept in the student's school file. It is not necessary to submit a copy of this form for verification.
