Verification is the process of confirming that a student's identified impairment and the associated activity limitations and participation restrictions requiring significant education adjustments meet criteria for one or more of the six Education Adjustment Program (EAP) categories. Verification is completed by the school team with parent consent and consultation.
Verification can proceed for students from Prep to Year 12 enrolled in state schools. In the EAP categories of hearing impairment, physical impairment and vision impairment, verification can occur for students attending non-state schools.
A verification request is completed by the school team, submitted to the EAP team by the principal, and assigned to a statewide verifier with experience and relevant qualifications in the EAP category. The statewide verifier considers the information and records a decision on the Adjustment Information Management System (AIMS) in OneSchool according to the departmental criteria.
Forms related to the verification process, and prompts for completing Criterion 2 for all EAP categories, are located on the Forms, documents and resources page.
Verification — state schools
All verification requests are submitted through AIMS in OneSchool.
A student who has a disability verified through the EAP is eligible for an EAP Profile on AIMS in OneSchool. The EAP Profile records the range and frequency of a sample of education adjustments made for the student.
Verification — non-state schools
Verification requests in the EAP categories of hearing impairment, physical impairment and vision impairment are submitted via email to the category-specific inbox. More information is available on the Forms, documents and resources page.
