The consent requests tile within the QParents application enables parents to access, populate and submit digital consent forms requested by their child’s school for:

  • student resource scheme participation agreement
  • online services consent
  • state school media consent
  • excursion consent.

Receiving consent request notifications

When a new consent request has been created for your child, you will receive a notification email. A notification will also display in your QParents account on the consent requests tile.

Actioning my child’s consent request in QParents

The consent requests tile on your QParents home screen will display the number of consent requests that are pending.

  1. Select view all from the consent requests tile.
  2. A list of pending consent requests will display.
  3. Select the pending consent request you wish to respond to and/or review.
  4. Scroll down the page to review the details of the consent request.
  5. Complete all of the required (*) fields
  6. Select the submit button.

Listing emergency contacts

Consent requests will allow only 1 emergency contact to be listed on the response.

Please note: The emergency contact details on the consent request are not visible to other QParents account owners (QPAOs) associated with the student.

Accessing my response in QParents

Once you have submitted your consent request through QParents, the consent requests tile will be updated with your response. You may be required to log out / log in to refresh the QParents application.

Changing my response to a consent request

You will not be able to change a submitted response to a consent request through QParents. You will need to provide your change of consent in writing to your school.

The send email button on the consent request will provide the email address of the contact staff member for that particular consent request. The school can then update the consent record and keep the evidence of the change of request.
