Queensland Curriculum and Assessment Authority—Queensland educational records are held by various agencies. Queensland Curriculum and Assessment Authority (formerly known as Queensland Studies Authority) lists the organisations to contact to obtain replacement copies. For those records available from the Authority, there are instructions on how to apply for a certified copy and a link to the Request for Replacement form.
